MIH is a condition in which the enamel of permanent first molars and sometimes incisors fails to fully mineralize during development due to systemic causes, resulting in incomplete and soft enamel. These children’s teeth begin to crumble as soon as they erupt due to chewing forces, causing severe hot and cold sensitivity. The primary goal of early clinical intervention is to reduce sensitivity and protect tooth tissue. To this end, the tooth surface is continuously supported with high-concentration fluoride varnishes and creams containing casein phosphopeptide-amorphous calcium phosphate (CPP-ACP). If crumbling has already begun, high-viscosity glass ionomer fillings with glass-like adhesion properties or stainless steel crowns (SSC) that completely protect the tooth should be applied quickly, as conventional composites cannot adhere to this weakened enamel.
