Root canals are not simply straight tubes running from top to bottom; alongside the main canal, they contain hundreds of microscopic networks of grooves, “lateral canals,” and “apical deltas,” much like the roots of a tree. The traditional method of compressing cold gutta-percha cones side-by-side (lateral condensation) cannot penetrate these microscopic lateral canals, allowing bacteria to continue residing there. In hot gutta-percha (thermoplastic filling) techniques, gutta-percha is heated inside the canal using specialized equipment, liquefying it, and then pressed vertically downwards. This material, having gained fluidity through heating, infiltrates under high pressure into all the microscopic lateral canals, ramifications, and perforations branching off from the main canal, completely sealing them in three dimensions. This hermetic (airtight and watertight) seal dramatically increases the long-term success of root canal treatment.
